- The VA cannot provide the services the Veteran needs.
- The VA cannot accommodate an appointment for the Veteran at the nearest VA facility within 30 days of the preferred treatment date.
- The Veteran lives more than 40 miles from the nearest VA facility.
- The veteran has to travel by air, boat, or ferry to get to the nearest VA facility.
- The Veteran faces an excessive burden in traveling to the nearest VA facility.
Steps in getting approved for the Veterans' Choice Program:
- The Veteran will need to gather information about any other health insurance coverage they may have.
- The Veteran will call 866-606-8198 to make sure they qualify and to schedule an appointment.
- The Veteran will be asked to enter their zip code, and first and last name.
- The Veteran will be asked for their address and the name of their preferred non-VA provider.
- The VA will check which of the Veterans ' needs are not able to be accommodated by the VA.
- The VA will check to make sure the Veteran is eligible for the program.
- The VA will reach out to the approved non-VA provider of the Veterans' choice to schedule their first appointment.
